Inputs stay in this page’s memory unless you explicitly save or export a draft. Reloading clears unsaved data. Device saves and exported files may contain business information. Use aggregate data; omit workers’ names and medical details.

Blank means “Not provided”. Enter 0 only when verified. Use one reporting month and the same workforce scope for incidents and hours.

1. Scope and accountability
2. Workforce and incidents

Hours actually worked include overtime; exclude paid non-work time. For the OSHA method, classify each recordable case once by its most serious outcome (G, H, I or J). First-aid-only cases and near misses are separate.

“Workdays lost during month” is a company metric, not OSHA’s calendar days-away count or a rate numerator. Recordability is determined by OSHA criteria, not simply whether a shift was missed.

3. Assurance and corrective actions

Closing balance = opening + raised − closed. The priority list below is a selected subset of your full action register. Status and overdue figures are assessed at month-end; a due date before that cut-off is overdue. Verify control effectiveness before closure.

4. Training and environment

Person-hours sum each attendee’s time. Recorded attendance hours take precedence. Otherwise the estimate is completed participants × full course duration; this excludes partial attendance. Assignment/completion totals count course participations, not unique workers.

Environmental measures

Company-reported quantities. Use consistent measurement boundaries and evidence. Waste classification follows applicable local rules; these fields do not determine legal compliance.

5. Management review

Definitions, interpretation and sources

This is a management reporting aid. The structure follows the review-and-improve approach in HSE HSG65 and ILO-OSH 2001: monitor performance, investigate, consult workers, assign actions and review their effectiveness. It is not an OSHA statutory filing, ISO certification or a substitute for local reporting requirements.

TRIR / TRC = (G + H + I + J) × 200,000 ÷ hours worked. DART = (H + I) × 200,000 ÷ hours worked. Days-away case rate = H × 200,000 ÷ hours worked. The 200,000-hour basis represents 100 full-time workers working 40 hours/week for 50 weeks. These are case rates; workdays lost are not the numerator. A monthly rate is normalised to this basis, not an annual forecast. Small monthly populations can produce volatile rates.

Inspection/audit coverage = completed ÷ planned × 100; ad-hoc work can exceed 100%. Action closure = closed ÷ (opening + raised) × 100. Training completion = completed course participations ÷ assigned participations × 100. These are administrative indicators defined by this tool. A zero denominator gives an undefined result. Counts and completion rates alone do not demonstrate control effectiveness; review findings, exposure, learning and worker feedback alongside them.
